HOW TO GET HELP FROM THE ALWAYS SUNNY FOUNDATION
Always Sunny Foundation is eager to help children in the Maryland and the Washington DC area through our Helping Hand program. This program is to help local families acquire special needs equipment for children with disabilities. We can help with walkers, wheelchairs, specially-designed adaptive bikes, home and vehicular ramps and other types of devices which are beyond the financial means of the family. Always Sunny Foundation will contribute up to $3,000.00 yearly per request for eligible candidates.
HOW TO APPLY FOR HELP
- Please write to us about your child and include the following information:
- Your name, parent or guardian:
- Your address:
- Contact phone number:
- Relationship to Child:
- Child’s name:
- Child’s age including date of birth:
- Child’s diagnosis:
- Type of health insurance, please list all:
- Number of dependents in the family along with ages and names:
Please submit the following documentation along with the above information:
- Letter(s) of verification from professionals (therapist, doctor, social worker) who are most familiar with your child’s needs. This letter should be current and clearly specify your child’s needs for the equipment requested and benefits of use. Please include as much detail as possible and provide the professional’s e-mail address, phone number and mailing address.
- A prescription from a medical professional (therapist or doctor) for the need of requested equipment.
- Letter from the applicant with a description of the child’s situation and how Always Sunny Foundation can help.
- Documentation of attempts to obtain other funding from insurance, other non-profit organizations, grants, loans, etc.
- Include letter of Insurance partial payment or denial.
- Has the family ever received assistance from Always Sunny Foundation in the past? If so, when and in what form?
- Include most recent tax return.
- A detailed description of the equipment requested, including brand name and all additional components.
- Three detailed, itemized quotes from suppliers that state the equipment, all additional components necessary to make the equipment a perfect fit for your child, and a total cost.
- Proof of ownership is required if the request is for a modification to a home or vehicle.
- Recent photo of the child.
If funding is approved, we do require photographs of the child with the equipment (preferably within one month of receiving the equipment). You will also need to sign the following. (These forms will be emailed to you once funding is approved)
- Release of Liability Form
- Disclaimer Form
- Authorization to Use Name and Likeness
